Within Mekor Shalom's Shana Tova Booklet,
there will be a section of Yizkor Remembrance Pages.

You may choose to include the names of deceased loved ones you would like to honor in this section of the booklet. Please complete the form below.

There is no charge or fee, and all submitted names will be listed.

Please consider a Yizkor Remembrance Pages donation of at least $18 as a tribute in memory of all of those whom you wish to include.
